Your First Visit

Pre-Registration:

At your first visit, we will ask you for information that we will need to take care of you. Rest assured this information will be kept confidential.
To make things easier at your first visit, registration forms may be printed and filled out in advance. They are in Acrobat (PDF) format and may be opened by clicking on the links on the Forms page.
The forms can be faxed to us at (901) 757-8960, mailed to us, or brought in with you to your first visit. Please call us with any questions.

Please bring your insurance card(s), photo ID, and co-pay with you to your first visit to see us.
As a courtesy to you, we will file your insurance for you. You will be responsible at the time of the visit for co-pays and deductibles as outlined by your insurance plan.

If your insurance company requires a referral, please make sure this referral has been made prior to your appointment. If you are not sure, please contact your insurance company to find out if a referral is necessary.

If allergy testing may be performed at your first visit, please do not take daily antihistamines for 5 days prior to this visit (for loratadine it is longer, as many as 10 days). Do not stop other medications such as medications for asthma, high blood pressure, cholesterol or diabetes.
Examples of daily antihistamines: Allegra (fexofenadine), Zyrtec (cetirizine), Xyzal (levocetirizine), Clarinex (desloratadine), or Claritin (loratadine).
Short acting antihistamines such as Benadryl should be held for 3 days.
Singulair (montelukast) should be held for 2 days.

If you are not sure or if you feel you cannot stop these medicines, continue them and Dr. Hanissian will discuss it with you when you come in. Testing can be scheduled for a later time.
